PruHealth Registration
To register to sell PruHealth print off and complete the following application forms and return them to:

Application forms can be faxed to PruHealth on 01786 405511.
Adviser Zone Registration
You can register online or create a username and password over the phone, please call PruHealth on 0800 092 1333 (Monday - Friday 8am - 8pm, Saturday 8am - 12pm)
If you have already registered and have a username and password, click on Log In above. Next time you can log on straight from the Home Page.
Broker Vitality Registration
Firms registering as a PruHealth agency in 2009 will automatically be registered for Broker Vitality, though the Broker Principal will have the opportunity to opt out of the following if they wish:
- Sales and Retention points earning category (rewards are limited to a maximum of Silver) AND/OR
- Individual Rewards (Hospitality, Health and Leisure rewards) AND/OR
- Business Rewards (SME leads, Marketing Support and Team Building Days) OR
- The whole Broker Vitality Programme
If you are the Broker Principal of a firm that is already registered with PruHealth, but not for Broker Vitality, you can download the Broker Vitality Registration Form as a PDF (56kb) and send it to your Account Manager or fax it to PruHealth on 0800 015 8900
UNIPASS
The
Adviser Zone also accepts the Unipass certificate. If you have one please click
on the Log On button on the top right hand menu to log onto
the website. If you would like to apply for a Unipass certificate, go to www.unipass.co.uk.
